Case Report

Several case reports suggest that topical 10% metformin cream may be a promising treatment for central centrifugal cicatricial alopecia (CCCA), particularly in patients unresponsive to standard therapies. In one case, a 69-year-old African American woman with stage 4a CCCA showed substantial hair regrowth after 6 months of daily metformin cream use, following five years of limited success with minoxidil, corticosteroids, and triamcinolone injections. Another patient, aged 54, also with stage 4a CCCA, experienced visible improvement within 4 months of adding daily topical metformin to an ongoing regimen of minoxidil and steroid injections.
